What is Flat Feet?
Flat feet, known medically as pes planus, is a condition where the arches of the feet are lower than normal or completely collapsed, causing the entire sole to make contact with the ground when standing. While some people have flat feet from childhood without any symptoms, others develop fallen arches over time and experience pain that affects their feet, ankles, knees, and even their lower back.
Acquired flat feet in adults most commonly develop due to weakness or dysfunction of the posterior tibial tendon, the main tendon responsible for supporting the arch. Factors such as ageing, weight gain, prolonged standing on hard surfaces, and wearing unsupportive footwear all contribute to progressive arch collapse.
Many people with flat feet experience no pain at all and require no treatment. However, when flat feet become symptomatic, they can cause aching along the inner foot and ankle, fatigue during walking, and difficulty with activities that require prolonged standing or walking.
Flat feet also change the alignment of the entire lower limb, causing the ankles to roll inward and the knees to shift into a knock-kneed position. This altered biomechanics can contribute to ankle, knee, hip, and lower back problems over time.

Assessment
We evaluate your foot arch height in both sitting and standing, assess ankle and foot joint mobility, test the strength of your posterior tibial tendon and intrinsic foot muscles, and examine your lower limb alignment from the hip to the foot.
Diagnosis
We determine whether your flat feet are flexible or rigid, identify any posterior tibial tendon involvement, and assess how your foot posture is affecting your ankles, knees, and lower back. This guides whether treatment focuses on strengthening alone or includes orthotic support.

Can flat feet be corrected without surgery?
In many cases, yes. Flexible flat feet, which are the most common type, often respond very well to physiotherapy focused on strengthening the muscles that support the arch. While the bone structure of the foot may not change, strengthening the posterior tibial tendon and intrinsic foot muscles can meaningfully improve arch height during activity and reduce symptoms. Surgery is typically only considered for severe, rigid flat feet that do not respond to conservative treatment.
Do I need to wear orthotics all the time for flat feet?
Not necessarily. Orthotics can provide helpful support during activities that place high demand on the feet, such as prolonged standing or running. However, relying on orthotics without also strengthening the foot muscles can lead to dependency. Our approach is to use orthotics where they are genuinely needed while progressively building the muscular strength that provides natural arch support.
Can flat feet cause knee or back pain?
Yes, flat feet can contribute to knee and lower back pain. When the arches collapse, the ankles roll inward, which changes the alignment of the knees and can increase stress on the knee joints and the lumbar spine. Addressing foot mechanics through strengthening and alignment correction can often improve knee and back symptoms that originate from poor foot posture.
