What is Runner's Knee / Patellofemoral Syndrome?
Patellofemoral pain syndrome, commonly known as runner's knee, is a condition characterised by pain around or behind the kneecap (patella). It occurs when the kneecap does not track properly in the groove of the thigh bone during bending and straightening, creating irritation of the cartilage and surrounding tissues.
This condition is one of the most common causes of knee pain in active adults and adolescents. Despite its name, it does not only affect runners and is frequently seen in people who cycle, hike, play sports, or simply spend long periods sitting with bent knees.
The pain typically worsens with activities that load the patellofemoral joint, such as squatting, climbing stairs, running, or sitting for prolonged periods with the knees bent. Many patients notice a grinding or crunching sensation behind the kneecap during movement.
The underlying causes are usually biomechanical rather than structural. Weakness in the quadriceps and hip muscles, poor foot mechanics, muscle tightness, and sudden increases in training load are the most common contributing factors.

Others may need a temporary break from running while building strength. Your physiotherapist will help determine the right approach for your situation.
Why does my knee hurt when I sit for a long time?
This symptom, sometimes called theatre sign, occurs because prolonged knee bending increases the pressure between the kneecap and the thigh bone. The position also places the kneecap cartilage under sustained compression. Straightening the leg periodically and strengthening the quadriceps muscles can help reduce this sitting-related pain.
How long does it take to recover from patellofemoral pain?
Most patients see significant improvement within six to twelve weeks of consistent physiotherapy and exercise. Full resolution and return to unrestricted sport typically takes three to six months. The key to successful recovery is addressing the underlying muscle weaknesses rather than simply managing pain.
