Heel pain is an umbrella term for any discomfort felt in or around the heel bone, and it is one of the most common foot complaints that brings patients to our clinic. While plantar fasciitis is the best-known cause, there are many other conditions that can produce heel pain, each requiring a different treatment approach.

The heel bears the brunt of your body weight with every step, absorbing forces of up to three times your body weight during running. This makes it vulnerable to a range of overuse and degenerative conditions, including fat pad syndrome, calcaneal stress fractures, Sever's disease in adolescents, nerve entrapment, and insertional Achilles tendinopathy.

Distinguishing between the different causes of heel pain is crucial because the wrong treatment can delay recovery or even make the problem worse. Pain under the heel, behind the heel, and along the sides of the heel each point to different structures and conditions that need targeted management.

What is the most common cause of heel pain?

Plantar fasciitis is the most common cause of heel pain in adults, accounting for approximately 80 percent of cases. It causes pain under the heel that is typically worst with the first steps in the morning. However, many other conditions can mimic plantar fasciitis, which is why a proper clinical assessment is important to ensure you receive the right treatment.

Can heel pain be a sign of something serious?

Most heel pain is caused by overuse conditions that respond well to physiotherapy. However, heel pain can occasionally indicate a calcaneal stress fracture, nerve entrapment, or other conditions that require specific management. Persistent heel pain that does not improve with basic self-care, or pain that is severe and came on suddenly, should be assessed by a healthcare professional.

Is heel pain different in children and teenagers?

Yes, heel pain in children and teenagers is often caused by Sever's disease (calcaneal apophysitis), a growth-related condition where the growth plate at the back of the heel becomes irritated. This is different from adult heel pain conditions and requires a modified treatment approach. It is common in active young people during growth spurts and typically resolves with appropriate management.

Why is my heel pain worse when I walk barefoot?

Walking barefoot on hard surfaces removes the cushioning and support that footwear provides, placing more direct stress on the heel structures. For conditions like fat pad syndrome and plantar fasciitis, this increased load on the heel can significantly worsen pain. Wearing supportive footwear indoors, even at home, can make a noticeable difference.
