What is Achilles Tendinopathy?

Achilles tendinopathy is a condition characterised by pain, swelling, and stiffness in the Achilles tendon, the thick tendon that connects the calf muscles to the heel bone. It is one of the most common overuse injuries affecting the lower leg, particularly in runners, recreational athletes, and people who are on their feet for extended periods.

The condition occurs when the tendon is subjected to loads that exceed its capacity to recover, leading to structural changes within the tendon fibres. Rather than a simple inflammation, Achilles tendinopathy involves a failed healing response where the tendon becomes disorganised and thickened, which is why rest alone rarely resolves it.

Achilles tendon pain typically comes on gradually and is often felt as stiffness first thing in the morning or at the beginning of exercise. Many people notice that the pain eases during activity but returns afterward, which can create a misleading impression that the problem is improving.

Without proper management, Achilles tendinopathy can become a stubborn, long-lasting condition that severely limits your ability to walk, run, or participate in sports. In some cases, a weakened tendon may be at increased risk of a complete rupture.

At Kinesio Rehab, we use progressive loading programmes that are the gold standard for Achilles tendinopathy treatment, rebuilding tendon strength and resilience so you can return to your activities with confidence.

Our Achilles Tendinopathy Treatment Process

1

Assessment

We assess the location and severity of your tendon pain, measure calf muscle strength and flexibility, and evaluate your ankle range of motion. For runners, we analyse your gait pattern to identify biomechanical factors contributing to tendon overload.

2

Diagnosis

We determine whether your tendinopathy is insertional (at the heel bone attachment) or mid-portion (in the body of the tendon), as this influences treatment approach. We also screen for other conditions such as retrocalcaneal bursitis or partial tendon tears.

3

Treatment Plan

We design a structured loading programme matched to your current tendon capacity, with clear progression criteria for advancing through each stage. Your plan accounts for your activity goals, whether that is returning to competitive running or simply walking without pain.

4

Active Treatment

Sessions focus on progressive tendon loading exercises, calf strengthening, manual therapy for associated muscle tightness, and sport-specific rehabilitation. We monitor your response to loading carefully and adjust the programme to ensure steady improvement.

5

Recovery & Prevention

We guide your return to full activity with a graduated increase in training load, provide ongoing tendon health exercises, and educate you on load management principles that prevent future episodes of tendinopathy.

Why Choose Kinesio Rehab for Achilles Tendinopathy

Achilles tendinopathy requires a precise, progressive approach, and at Kinesio Rehab, we follow the latest evidence-based protocols for tendon rehabilitation. Our physiotherapists understand that tendons respond to carefully dosed loading rather than rest, and we design programmes that build tendon capacity safely over time.

We provide gait analysis for runners and sport-specific rehabilitation for athletes, ensuring that the treatment addresses the demands you place on your tendon. Our clinic is equipped with the tools needed for accurate strength testing and progressive loading.

We stay current with tendon research so that you receive the most effective treatment available.

Achilles Tendinopathy — FAQs

Why does my Achilles tendon hurt more in the morning?

Morning stiffness and pain are hallmark features of Achilles tendinopathy. During sleep, the tendon shortens slightly as the foot relaxes into a pointed position. When you take your first steps, the tendon is suddenly stretched under load, producing pain. This morning stiffness typically improves within the first 10 to 15 minutes of walking.

Should I stop running if I have Achilles tendinopathy?

You do not necessarily need to stop running entirely, but you will likely need to modify your training load. Continuing to run through significant pain can worsen the condition, while complete rest does not stimulate the tendon healing process. We can help you find the right balance by adjusting your running volume and intensity while you rehabilitate the tendon.

How long does Achilles tendinopathy take to recover?

Achilles tendinopathy typically requires three to six months of consistent rehabilitation to achieve a full recovery. Tendon tissue adapts slowly to loading, so patience and adherence to the exercise programme are essential. Most patients notice meaningful pain reduction within the first six to eight weeks.
