What is Whiplash Injury?

Whiplash is a neck injury caused by a sudden, forceful back-and-forth movement of the head, much like the cracking of a whip. It most commonly occurs during rear-end car accidents, but can also result from sports collisions, falls, or any impact that causes rapid acceleration and deceleration of the neck.

During a whiplash event, the neck is forced beyond its normal range of motion in a fraction of a second. This can damage the muscles, ligaments, tendons, discs, and joints of the cervical spine.

The soft tissues are stretched or torn, and in more severe cases, the joints between the vertebrae can also be injured.

Symptoms of whiplash often do not appear immediately after the incident. It is common for pain and stiffness to develop 12 to 24 hours later, and sometimes symptoms continue to worsen over the following days before they begin to improve.

The severity of whiplash varies widely. Some people recover within a few weeks, while others develop chronic symptoms that persist for months or even years.

How soon after a car accident should I start physiotherapy?

Ideally, you should begin physiotherapy within the first one to two weeks after a whiplash injury. Research strongly supports early active treatment over prolonged rest and collar use. The sooner you start guided movement and rehabilitation, the lower your risk of developing chronic neck pain. If you are experiencing severe symptoms, see a doctor first to rule out fractures, then begin physiotherapy promptly.

How long does whiplash take to heal?

Most whiplash injuries improve significantly within six to twelve weeks with appropriate treatment. Mild cases may resolve in two to four weeks, while more severe injuries can take three to six months. Approximately 20 to 40 percent of people develop some degree of chronic symptoms, which is why early and thorough rehabilitation is so important for preventing long-term problems.

Should I wear a neck collar after whiplash?

Current evidence recommends against prolonged neck collar use after whiplash. While a collar may provide short-term comfort in the first day or two, wearing one for longer actually slows recovery by weakening the neck muscles and promoting stiffness. Early active movement within comfortable limits is far more beneficial for healing.

Can whiplash cause long-term problems?

If not properly treated, whiplash can lead to chronic neck pain, persistent headaches, and ongoing stiffness. Factors that increase the risk of long-term problems include delayed treatment, high initial pain levels, and the presence of neurological symptoms. Early physiotherapy significantly reduces the likelihood of developing chronic symptoms.
