What is Muscle Strain?

A muscle strain occurs when muscle fibres are stretched beyond their capacity and partially or completely tear. These injuries are graded from mild stretching of a few fibres to a complete rupture of the entire muscle belly.

Muscle strains are extremely common among Malaysians who participate in sports such as badminton, football, futsal, and running, as well as those who perform sudden bursts of activity without adequate warm-up. The hamstrings, quadriceps, calf, and groin muscles are the most frequently affected areas.

Symptoms typically include a sudden sharp pain at the moment of injury, followed by swelling, bruising, and difficulty using the affected muscle. In severe tears, you may notice a visible gap or lump in the muscle where the fibres have separated.

The severity of a muscle strain determines how long recovery takes, ranging from a few weeks for a mild grade-one strain to several months for a complete grade-three tear. Without proper rehabilitation, scar tissue can form in a disorganised pattern that leaves the muscle weaker and more vulnerable to re-injury.

How long does it take for a muscle strain to heal?

Recovery time depends on the severity of the strain. A mild grade-one strain may heal in two to four weeks, a moderate grade-two strain typically requires six to twelve weeks, and a severe grade-three tear can take three to six months or longer. Proper physiotherapy can accelerate recovery and reduce the risk of re-injury.

Should I use heat or ice on a muscle strain?

In the first 48 to 72 hours after injury, ice is generally recommended to help manage swelling and pain. After the initial acute phase, gentle heat can be used to promote blood flow and relax the surrounding muscles. Your physiotherapist will guide you on the most appropriate approach for your specific injury stage.

Can I still exercise with a muscle strain?

You should avoid activities that load the injured muscle during the early healing phase. However, complete rest is not recommended for most strains. Your physiotherapist will prescribe safe exercises that maintain fitness and mobility without aggravating the injury, gradually progressing the load as the muscle heals.
