The hip labrum is a ring of firm cartilage that lines the rim of the hip socket, helping to deepen the joint and provide stability, cushioning, and a seal that keeps the joint fluid in place. A hip labral tear occurs when this cartilage is damaged, either through injury, repetitive movements, or structural variations in the hip joint.

Labral tears are a common cause of deep groin pain and clicking or catching sensations in the hip. They are frequently seen in athletes who perform repetitive twisting, pivoting, or deep squatting movements, such as football, badminton, martial arts, and dance.

However, they can also occur in non-athletes due to hip joint impingement or gradual wear.

Many people with a hip labral tear describe a deep ache in the groin or front of the hip that worsens with prolonged sitting, squatting, or twisting movements. Some experience a sharp catching sensation during certain hip positions, and the hip may feel unstable or as though it is giving way during activity.

The condition is often associated with femoroacetabular impingement, where the shape of the hip bones causes abnormal contact between the ball and socket during movement. This bony impingement can gradually damage the labrum over time, even without a single traumatic event.

Can a hip labral tear heal without surgery?

While the labrum has limited blood supply and may not fully heal on its own, many patients with labral tears experience significant symptom relief and return to full activity with physiotherapy alone. Strengthening the muscles around the hip and correcting movement patterns can reduce the stress on the labrum and eliminate symptoms. Surgery is typically considered when conservative treatment has not provided adequate relief after several months.

What activities should I avoid with a hip labral tear?

Activities that involve deep hip flexion, forceful twisting, or repetitive impact tend to aggravate a labral tear. Deep squats, prolonged cross-legged sitting, high-impact running, and sports with sudden direction changes may need to be temporarily modified. What is femoroacetabular impingement and how does it relate to labral tears?

Femoroacetabular impingement, or FAI, is a condition where extra bone along the hip ball or socket causes the bones to contact abnormally during movement. This repeated abnormal contact can pinch and gradually damage the labrum over time. Addressing impingement-related movement restrictions is an important part of labral tear rehabilitation to prevent ongoing damage to the cartilage.

How long is rehabilitation after hip labral surgery?

Rehabilitation after hip arthroscopy for a labral tear typically takes four to six months, with most patients returning to daily activities within six to eight weeks and sport between four and six months. The rehabilitation follows a phased approach with specific milestones for range of motion, strength, and functional ability.
