An elbow fracture involves a break in one or more of the three bones that form the elbow joint: the humerus, radius, or ulna. These fractures commonly result from falls onto an outstretched hand, direct blows to the elbow, or high-impact injuries during sport or road accidents.
The elbow is one of the most challenging joints to rehabilitate after a fracture because it is highly prone to stiffness. Even a short period of immobilisation can lead to significant loss of bending and straightening movement, and scar tissue forms readily around the joint.
Treatment for elbow fractures may involve a cast or splint for stable fractures, or surgery with plates, screws, or wires for displaced or complex fractures. Regardless of how the fracture is managed, physiotherapy is essential for regaining movement and function afterward.

When should I start physiotherapy after an elbow fracture?
The timing depends on the type of fracture and how it was treated. For surgically fixed fractures, gentle range of motion exercises often begin within one to two weeks after surgery. For fractures treated with a cast, physiotherapy starts once the cast is removed, typically after four to six weeks. Early intervention is crucial because the elbow stiffens rapidly during immobilisation.
Will I regain full elbow movement after a fracture?
Many patients regain functional range of motion, though some may have a small residual loss of full extension or flexion, particularly after complex fractures. The functional range needed for most daily activities is achievable with dedicated rehabilitation. Starting physiotherapy early and being consistent with exercises gives you the best chance of recovering maximum movement.
Why is my elbow so stiff after the fracture healed?
The elbow joint is particularly prone to stiffness after injury because scar tissue and adhesions form readily within and around the joint capsule during immobilisation. The joint surfaces and surrounding soft tissues respond to even short periods of inactivity by tightening up. This is why physiotherapy with progressive mobilisation is so important after an elbow fracture.
Can I do strengthening exercises during elbow fracture recovery?
Strengthening exercises are introduced gradually based on your healing stage. In the early weeks, the focus is on range of motion rather than strength. Gentle grip and forearm exercises may begin around four to six weeks, with more demanding strengthening introduced once the fracture has sufficiently healed, typically around eight to twelve weeks post-injury.
