Rotator Cuff Retear: Prevention and Rehabilitation After Repair Surgery
Rotator cuff retear is one of the most common complications following rotator cuff repair surgery, with reported retear rates ranging from 20% to 70% depending on tear size, tendon quality, patient age, and rehabilitation approach. A retear does not always mean the surgery has failed -- some patients with structural retears remain functionally strong and pain-free -- but prevention is always preferable to managing a second failure. Understanding the risk factors and following an evidence-based rehabilitation programme are the two most important things you can do to protect your repair. Why Retears Happen
The repaired tendon is at its weakest in the first 6-12 weeks after surgery. During this period, the sutures hold the tendon to bone while biological healing (tendon-to-bone integration) slowly progresses. Retears most commonly occur because of premature loading -- lifting too heavy too soon, returning to overhead activity before the tendon has healed, or performing exercises that exceed the repair's tolerance.
Patient-related risk factors include: age over 65 (tendons heal more slowly), large or massive tears (greater than 3 cm), fatty infiltration of the rotator cuff muscles (visible on pre-operative MRI, indicating chronic degeneration), poor tendon quality at the time of repair, diabetes (impairs tissue healing), and smoking (reduces blood flow to the tendon). Surgical factors such as the repair technique (single-row versus double-row) and the number of anchors used also influence retear risk, though these are beyond the patient's control.
Importantly, being too cautious can also be harmful. Excessive immobilisation leads to shoulder stiffness (adhesive capsulitis), muscle atrophy, and prolonged weakness -- creating a weak, stiff shoulder that functions poorly even if the repair remains intact. The goal of rehabilitation is to find the optimal balance between protection and progressive loading.
Rehabilitation Principles for Retear Prevention
Phase-based progression is the cornerstone of safe rehabilitation. The four phases align with the biology of tendon healing:
Phase 1 (weeks 0-6): Maximal protection. The arm is in a sling with an abduction pillow. Only passive and gentle active-assisted range of motion is permitted. Pendulum exercises, passive forward flexion (physio-assisted), and scapular setting keep the shoulder mobile without loading the repair. No active shoulder elevation. No lifting with the operated arm.
Phase 2 (weeks 6-12): Active motion. Active range of motion begins -- lifting the arm under its own power. Active-assisted exercises transition to fully active movement. Light isometric rotator cuff exercises (pressing gently into a wall) initiate muscle re-engagement. The critical rule: no resistance band or weight training for the rotator cuff until at least week 12.
Phase 3 (weeks 12-20): Progressive strengthening. Resistance band exercises for internal and external rotation begin at low loads and are progressed gradually. Scapular strengthening (rows, prone Y-T-W) is intensified. Closed kinetic chain exercises (wall push-ups, planks) load the shoulder through compression rather than tension, which is safer for the healing tendon. Strengthening is progressed based on pain response -- no exercise should provoke more than mild discomfort (2/10 on a pain scale) that settles within 24 hours.
Phase 4 (months 5-9): Functional loading and return to activity. Sport-specific or work-specific training begins. Overhead strengthening, plyometric loading (for athletes), and simulated work tasks test the repair under real-world conditions. Return to full activity is guided by objective strength testing -- external rotation and abduction strength should reach at least 80-85% of the uninvolved side.
Key Exercises for Long-Term Tendon Health
Eccentric rotator cuff training -- slowly lowering the arm against resistance -- is one of the most evidence-supported approaches for building tendon resilience. Eccentric loading stimulates collagen remodelling and increases tendon stiffness and load tolerance. Your physiotherapist will introduce eccentrics around week 14-16 using resistance bands or cables, starting with 3 sets of 10 repetitions at a controlled 3-4 second lowering tempo.
Scapular muscle endurance is equally important. The lower trapezius and serratus anterior control scapular upward rotation -- the movement that creates space for the rotator cuff tendons during overhead activity. Weakness in these muscles causes the scapula to "wing" or tilt forward, impinging the rotator cuff and increasing retear risk. Exercises like wall slides, side-lying external rotation with scapular focus, and prone horizontal abduction build the endurance these muscles need for sustained overhead function.
Posterior shoulder stretching (sleeper stretch, cross-body adduction stretch) maintains glenohumeral internal rotation range, preventing the posterior capsule tightness that shifts the humeral head forward and stresses the repaired cuff during overhead movements.
Lifestyle Modifications That Protect Your Repair
Beyond the exercise programme, several habits reduce retear risk. Sleep position: sleep on the opposite side or on your back with a pillow supporting the operated arm for the first 3-4 months. Avoid reaching behind the back (e.g., tucking in a shirt, reaching for a seatbelt) for the first 12 weeks, as this position combines extension, internal rotation, and adduction -- the most stressful combination for a repaired supraspinatus. Gradual return to driving: steering demands active shoulder control, so wait until your surgeon and physiotherapist clear you (typically 6-8 weeks for automatic vehicles). If you smoke, cessation significantly improves tendon healing rates.
